Fix relays re-running slow PHP requests every 20s

The 20s idle timeout on php-fpm connections also fired while PHP was still handling a request. The relay then re-sent the request on a new connection every 20s, so slow pages ran many times and filled the PHP pool.

The read loop now keeps waiting if the timeout fires while a request is in flight. Idle connections are still closed after 20s.

V4: Support uninformed egress proxy clients

This change includes support in web-server-v4 images to rewrite the /etc/hosts file with the address of the core server for all hostnames given in metadata entry PROXY_HOSTNAMES.

V4: Removal of default php request_terminate_timeout

This was previously set to 60 seconds by default. This means that any response that took longer than 60 seconds to construct was canceled. This setting was initially used to ensure that no PHP-FPM requests would hang indefinitely, with 60 seconds chosen as a reasonable upper bound for a request. However, admin requests frequently take longer than this and so become unprocessable. Since a client that closes a connection already yields a FastCGI “abort request”, removing this default allows the timeout to be correctly controlled by setting a timeout in the data plane using web controls. Therefore, this default is removed.

V4: Fix rsync permissions bleeding from shadow

The shadow mount would allow chown and chgrp to be performed, which rsync would automatically do. This change supresses chown and chgrp through /shadow/www, ensuring that the ownership remains www-data:www-data.
